A Broken Guidewire Fragment Lodged in Superior Vena Cava during Subclavian Venous Catheterization

Abstract

|
Mechanical complications of central venous catheterization are important, because their effects are usually immediate and contribute to increased length of stay, increased hospital costs, need for subsequent interventions, and mortality rate. These include failure to place the catheter, pneumothorax, arterial puncture, pulmonary embolism, air embolism and dysarhythmia. Breakage of a guide wire during central venous catheter insertion has been very rarely reported. We report a case of breakage of guidewire during subclavian venous catheterization in a 71-year-old women with post-operative ileus, parkinsonism and malnutrition. Guidewire was inserted into the patient``s subclavian vein. After insertion of subclavian venous catheterization, the guidewire was hardly extracted and broken. We found a piece of broken guidewire in superior vena cava, and the length of the broken guidewire was about 1.5 cm. The broken guidewire was not removed to avoid additional vessel damage. After 24 months of follow up, she did not present any specific symptoms and complications.
